Name/Surname change certificate

Required Documents:

1. Click here [Download] to download the petition form.
2. Official certificate of name/surname change
3. Thai Citizen ID or Passport
4. House Registration (for Thai citizens)
5. Submit the approved petition to the Registration Office:
   • Suvarnabhumi Campus: SM116, Counter 3
   • Hua Mak Campus: P Building, Counter 4

Important Notes:

• The University does not recognize name changes made after graduation.
• Upon request, the University may issue an official certification acknowledging the name change.
